Registered Nurse - Home & Community Based Care Outpatient

The Home & Community Based Care, Community Adult Day Health Care program partners with community adult-day health care sites to support Veterans in need of assistance with activities of daily living, isolated, would most likely be in a nursing home but funding/resources are not currently available, or when their caregiver is experiencing burden. The Registered Nurse plays a critical role in linking Veterans to appropriate community facilities and providing complex case management.
