PCMHI Collaborative Care Management (PCMHI CoCM) is a condition-specific (e.g., depression), protocol-driven service to collaboratively assist Patient Aligned Care Team (PACT) providers in treating common mental health and health behavior concerns, often in the context of pharmacotherapy provided by Primary Care Providers (PCPs). The PCMHI CoCM RN is responsible for providing competent, evidence-based care to assigned patients.
